I can't help you with the first two problems, but in regard to the third one I believe the issue is that it's not possible to grant kingdoms to republican and theocratic characters (and you can also only ever give them a single duchy). If you are a republic, you will create a title of "most serene republic" which same level as "kingdom", and if you found an empire, you will get the title euqal to empire, but remain the same form of government (republic), you cant really switch government form in ck2 with normal method unless game glitch/bug, etc.
